Bullseye has a tumor in his brain that makes him see every day people as Daredevil, and he starts to kill them all.

Matt Murdock appears on The Tom Snyder Show when an emergency broadcast interrupts the program with news that Bullseye, the deadly assassin, has escaped from Midtown Hospital just before undergoing surgery.

Later at the hospital, Daredevil and Lieutenant Manolis examine the murder scene in the operating room. Dr. Glosson explains that Bullseye has a cancerous growth in his brain, creating pressure that causes intense headaches and hallucinations. The doctor believes the tumor was responsible for Bullseye’s emotional collapse at Coney Island, noting that during his episodes he perceives everyone as Daredevil.

On the streets, Bullseye sees devils everywhere and is soon reported attacking and killing civilians. This news interrupts the discussion at the hospital. Daredevil tracks Bullseye to a Times Square movie theater, where he is brutally assaulting patrons. Their fight escalates until Bullseye takes a moviegoer hostage and hurls a knife at Daredevil. Daredevil tries to catch it, but the blade strikes his chest and drops him. Bullseye escapes with the hostage, forcing him to take him to his apartment.

Back at the theater, Manolis oversees the injured as Daredevil is taken away by ambulance. Daredevil realizes his only lead is a heavily medicated throat lozenge -- one of the hostages must suffer from a severe throat condition. Using his heightened senses, he isolates the sound of a cough and the scent of the lozenge, eventually tracking Bullseye and the hostage to the subway.

In the subway, the overwhelming noise disables Daredevil’s senses, allowing Bullseye to savagely beat him. Refusing to give up, Daredevil rallies and unleashes a brutal counterattack that knocks Bullseye unconscious. Too weak to pull him from the tracks as a train approaches, Daredevil struggles with the decision of whether to let his enemy die. In the end, he saves Bullseye.

Later at the hospital, Daredevil defends his choice to Manolis, explaining that the law exists to restrain men like Bullseye. He admits, “I wanted Bullseye to die. I detest him, but I’m not God, I’m not the law, and I’m not a murderer.” Manolis warns him that the next time Bullseye kills, it will be Daredevil’s fault. Moments later, the hospital announces that Bullseye’s operation was a success.
